He is survived by: his wife Evelyn Hornick; his sisters, Betty Giovanazzi of Homer Glen, IL and Aggie Mitchell of Roswell, GA; his children, Theresa Dostalik of Monaca, PA and Marty Dostalik (Kari) of Urbandale, IA; his daughter-in-law Becky Dostalik of Walnut, IL; his grandchildren, Ruth Vohland (Isaac) of Walnut, IL, Ben Dostalik of Walnut, IL, Adam Dostalik of Indianapolis, IN and Haley Dostalik of Houston, TX; and his great grandchildren, Livia, Audra and Jacob Vohland of Walnut, IL. He is also survived by several nieces and nephews.
