She was predeceased by: her father Joseph Anthony Zak, Sr.; and her grandparents, Arthur, Carol Olsen Voorhees, Carl and Julie Zakrzewski.
She is survived by: her mother Carolee Voorhees Zak of Flemington, NJ; her husband George C. Campbell, IV of Hellertown, PA; her brother Joseph Anthony Zak, Jr. (Kimberly Guarriello) of Flemington, NJ; her daughter Rachel Cozze; and her grandson Chase Andrew Cozze.
