She is survived by: her husband Ray; her son Tom Slattery (Joanna) of Madison; her grandchildren, Shelby and Marley Slattery; her sister Donna Daughenbaugh Phillips (Steve) of Lenoir, NC; her nieces, Sarah Keister and Elizabeth Keister; her great-nieces, Peyton Simmons and Kaitlyn Simmons; her niece Kelly Antonuccio (Sam) of Brockway; her great-nephews, Hunter Antonuccio and Dylan Antonuccio; her nephew Edwin Welch of Emporium, PA; and her great-nieces and great-nephews, Zack Dickey, DeHaven Welch, Brooke Welch and Cheyenne Welch.
