He is survived by: his children, Jerry Provin Jr. (Scott) of Jacksonville, FL, Teresa Morgan (Chris) of New Hampshire, Karen Trimble (Chris) of Jacksonville, FL and Elizabeth Ransom (Misty) of DuBois; his granddaughter Rebecca Figueroa (Gerardo) of New Hampshire; and his great grandchildren, Brooklynne Shields, Beau Shields, Charlie Rose Figueroa, Oliver Figueroa and Penelope Figueroa.
