He is survived by: his wife Donna May Hicks Hayes; his daughters, Patricia and Danielle; his grandchildren, Lydia, Gabby Polaski, Wyatt, Isaac Hayes, Axel Hayes-Roberts, Daelynn, Addison, Emalee and Kaylee Dinsmore; and his great grandchild Aurora Polaski.
