He is survived by: his wife Doris Ann Kozak (Kantoski); and his children, Michael R. Kozak Jr. (Nicole) of Hughesville, PA, Daniel J. Kozak (Jenny) of Montoursville, PA, Douglas J. Kozak (Lauren) of York, PA, David J. "Humm" Kozak (Kimberly) of Mechanicsburg, PA and Michele R. Gula (William) of Bluffton, SC. He is also survived by eight grandchildren; and four great-grandchildren.
