He was predeceased by: his wife Mary Jo Sirianni Kostiak; and his parents, Stephen Kostiak and Mary Wahal Kostiak. He is survived by: his children, Lorraine Ondrako (Richard), Rosann Fields (Robert) of Witherbee, NY and Stephen Kostiak (Julie); his grandchildren, Emma Holden (Marty), Stephen Kostiak, Jr. and Lily Rose Fields; his great grandchildren, Jameson and Cameron Holden; and his siblings, Paul Kostiak and Delores Brophy. He is also survived by several nieces and nephews.
