He was predeceased by: his parents, Robert Ballard and Clover Ballard (Urban). He is survived by: his wife Judy A Ballard (Mellott); his stepdaughters, Laura L. Schwenninger (Wayne), Joyce L. Hershey and Rebekah L. Deitz (William); his brother Scott J. Ballard (Barbara K. Ballard); and his nephew Daniel A. Ballard.
