He was predeceased by: his parents, Stanley Surmacz and Mary Surmacz (Cheplic); his brothers, Theodore (Virginia) and Eugene Surmacz (Frances); and his parents-in-law, Frank and Kathryn Shula.
He is survived by: his daughter Stacy Barkovich; and his granddaughters, Haley and Kara Barkovich. He is also survived by several nieces and nephews.
