He is survived by : his wife Sarah E. Coffield Shape; his son Leonard Prusakowski, Jr. of Claysville; and his stepchildren, John Shape (Kathy) of Cassville, Joe Shape of Claysville, Jim Shape (Darlene) of Claysville, Sue Yaworski (Bruce) of Claysville and Dawn Greenleaf (Justin) of Hammonton, NJ. He is also survived by many grandchildren, great-grandchildren, and nieces and nephews.
