She was predeceased by: her parents, Robert Disque and Stella Disque (Budzinski); her husband Allen B. Cummings; and her brother Jim Disque. She is survived by: her sons, Eric (Beth) of Polk and Joshua of Lavalette; her grandchildren, Cody, Tara, Erica and Braden; and her siblings, Pete Disque (Patty), Peggy Carlson (Greg), Teri Middleton (Jack) and Faye Marsh. She is also survived by great grandchildren.

Memorial contributions may be made to the Venango VNA Foundation, 491 Allegheny Blvd., Franklin PA 16323.
