He was predeceased by: his parents, Joseph Polacek and Clara Polacek (Urbassik); his wife Anna E. Polacek (Hegadus); his brother Robert J. Polacek; and his brother-in-law Richard Kelsey.
He is survived by: his children, Stephen J. Polacek (Laurie Skelly) of Johnstown, Eva Ann Murray (Timothy) of Jenners, PA, Julie Ann Doell and Jennifer Ann Stutzman (Greg Mattes) of Johnstown; his grandchildren, Victor Lord (Cheyanne), Nadine Lord, Chloe Doell, Logan Doell and Glenn Warren Stutzman; his stepgrandsons, Kenneth and Aaron Frazier; his siblings, Elizabeth Kelsey and Charles Polacek (Brenda Warner); his sister-in-law Evelyn Polacek (Bronosky); and his friend Norm Verhovsek. He is also survived by numerous nieces and nephews.
In lieu of flowers, donations can be made in memory of Joseph to his family.
