He is survived by: his children, Kimberly Tamburro (Danny) of North Versailles, Karen Forkovitch-Homick of Irwin and Daniel Forkovitch, Jr. of Gibbon Glade; his grandchildren, Megan Beers (David, Jr.), Mason, Lawren Forkovitch, Abigail Homick, Daniel Tamburro, Jr. and Nathan Forkovitch; his great grandchildren, Danielle, Maura, Lily and David III; and his sister Carol McKinney. He is also survived by nieces and nephews.
