He was predeceased by: his father Walter W. Cook; his wife Kathryn Jane Cook (Henry); and his sisters, Arlene Fridley, Maxine Marsh, Beulah "Boots" Brownell, Betty Uplinger, Margaret Burkett and Lilly Mae Johnson.
He is survived by: his children, Connie L. Crytser, Kenneth W. Cook (Betty Lou Bithell) and Terry L. Cook (Denise Patton); his grandchildren, Michael Crytser (Jen), Kenneth W. Cook, Jr. (Kelley), Matt Cook (Kevin), Robyn Rhodes (Dwayne), Terry L. Cook, Jr. (Carrie) and Tracy Burke (Reese); his great grandchildren, Jillian Bruni-Crytser, Kara Cook, Kaci Yensan (Tyler), Drew Cook, Isaac, Jacob, Kendal Cook, Jennica Bowling and Amara Burke; his sister Sara Rosio; and his siblings-in-law, Anthony Christian (Marilyn) and Kathy Johnson (Christian). He is also survived by many cousins, nieces, and nephews.
Although flowers are appreciated, the family would ask that donations be made to the local American Cancer Society or the organization of the donor's choice.
