He was predeceased by: his parents, Arcangelo Azzolina and Caroline Ballato Azzolina; his siblings, Theresa Caponigro, James Azzolina, Jennie Reda and Mary Azzolina; and his brother-in-law Dominick Cappellano.
He is survived by: his wife Carmella J. Cappellano Azzolina; his children, Tracy L. Muth (Stephen) and Joseph A. Azzolina (Brittany); his grandchildren, Brittany (Josh), Megan, Kyle, Dante and Marco; his sister Sadie Azzolina; and his sister-in-law Phyllis Cappellano.
