He is survived by: his daughter Tasha Howard (Andy); his grandchildren, Drew Howard (Shelby Claypool), Dustin Howard (Lauren Morse), Damian Cook (Abby), Chase Cook, Kaylynn Cook, Waylon, Charlotte and Hadlee; his sisters, Peggy Cook (Chic) and Candy Rix; his siblings-in-law, Marie Cook, Kathi Langworthy Taylor, Jane Langworthy Haymach, Maze Langworthy Peters, Steve Langworthy (Mary) and Denny Langworthy (Mindy); and his son Dale Cook. He is also survived by several nieces and nephews.
