He was predeceased by: his parents, Gerald Phillips and Barbara Ann Phillips (Roberts); and his brother Robert Phillips. He is survived by: his wife Janice; his brothers, Thomas Phillips (Theresa), Charles Phillips, Daniel Phillips (Mirna Deleon) and Gerald G. Phillips (Amy); his close friends, Jade Sarver, Nate Hawthorne, Heaven Sarver, Zain Hawthorne and David Barrick; and his sister-in-law Michelle Phillips. He is also survived by five nephews, two nieces, and one grandniece.
