He was predeceased by: his parents, Douglas C. Forde and Georgina Mesa. He is survived by: his wife Rita Forde (Schick); his siblings, Raymond and Julie Fernandez; his niece Jasmine Parker; his siblings-in-law, Edward J. Schick (Anne) and Joan G. Schick; and his caregiver Lia. He is also survived by many cousins, nieces, nephews and friends.
