He was predeceased by: his parents, Levi Perkins and Alice Perkins. He is survived by: his wife Elizabeth Ann; his sister Shirley Root; his sons, Dr. Thomas Perkins (Teya) and Timothy Perkins; his grandchildren, Tyler (Jenna), Ashlyn, Carter, Leah, Parker, Liliana, Logan, Harlow and Maisey Perkins; and his great grandchild Darby Perkins.
In lieu of flowers, the family kindly requests donations to St. Mark the Evangelist Church or the Lawrence Park Fire Department in his memory.
