He was predeceased by: his parents, Michael Sullivan and Mary Sullivan (Corrigan). He is survived by: his wife Ellen Sullivan (Simodejka) of Frackville; his children, Thomas B. Sullivan (Melinda) of Huntingdon, Tara Campton (Tom) of Boiling Springs, Trisha Przyuski (John) of Center Valley, Tammy Murphy (Steve) of Dillsburg and Timothy M. Sullivan (Brandy) of Fountain Springs; his siblings, Patricia Giuliani (Max) of St. Augustine and Michael Sean Sullivan (Geoff) of Palm Coast, Florida.; and his grandchildren, Ryan, Aaron, Evan, Mason, Grace, Jack, Declan, Liam, Rylee and Molly. He is also survived by many nieces, nephews, cousins, great-nieces, and great-nephews.
In lieu of flowers, the family asks that memorial donations/checks be made payable to the "Thomas M. Sullivan Scholarship" and sent to the funeral home at 501 W. Washington Street, Frackville, PA 17931.
