He was predeceased by: his parents, Calvin Eugene Hazlett and Mary Barnicle Hazlett; the mother of his children Barbara Ann Rhodes Hazlett; and his siblings, Kenneth J., Howard W. Hazlett, Virginia Wagner, Dorothy Tingley, Janet Pierce, Charlotte Woods and Marilyn Hazlett.
He is survived by: his children, Rose Miller (Mike) of Franklin, Brenda Josephs Simanteris of Firestone, Jenny Lyn Hazlett (Rob Gifford) of Brush, Tammy Lewis (Peter) of San Francisco, Charles Hazlett of Franklin, PA. and Rex Hazlett of Franklin, PA.; his close friends, Becky Crawford and Art Borger (Leslie); and also, Angie Crawford (Ralph) and Stephanie Crawford of Cooperstown. He is also survived by his 13 grandchildren and numerous great grandchildren as well as a multitude of nieces and nephews.
