She was predeceased by: her parents, John Affleck and Frances Noon; and her brother Jack Affleck. She is survived by: her children, Edward P. Durkin III, Kevin P. Durkin, Amy L. Thoma and Brian P. Durkin (Laura); her grandchildren, Kevin, Eddie, Kerianne, Jimmy, Brian, Mackenzie, Sam, Shannon, Jack, Colin, Ally and Grace; and her sister Joyce Preston.
In lieu of flowers, the family kindly requests donations to be made in Carolyn's memory to The Breathing Room Foundation in Jenkintown.
