He was predeceased by: his parents, Jan Raymond and Linda Ann Perkins (Long). He is survived by: his wife Donna L. Perkins (Barnard); his son Jan Tucker Perkins of Greensburg; his sister Katey Miller (Dan) of Irwin; his uncles, Christopher Long and Buck Long; his aunt Melissa Hill; and his niece Morgan Miller. He is also survived by nieces and nephews.
