She was predeceased by: her parents, James Falcone and Catherine Falcone; her husband Eugene Maciak; and her sister Dolores Zimmer. She is survived by: her children, Christine Isaacs (Jason), James Maciak (Katherine) and John Maciak (Deborah); and her grandchildren, Ashley Rout (William), Ryan (Jenna), Joshua, Nathan, Hannah, Noah, Lucas Maciak, Amber Revers (Edward), Aimee Briskin (Isaac) and Nicholas Isaacs. She is also survived by nieces and nephews.

Memorial contributions may be made to Our Lady of Peace School.
