He was predeceased by: his parent Michael J. Sr. He is survived by: his parent Catherine M. Kolackovsky, Sr. (Fabry); his wife Phyllis; his daughters, Kathryn M. Prieto (Victor) of Wilmont, NH, Ann Michele Leary of Warrenton, VA and Carol J. Smith (Matthew) of Harrisburg; and his siblings, John Kolackovsky of Rockport, MA, Delores Kolackovsky of Old Bridge, NJ and Patsy Rossi of West Creek, NJ.

In lieu of flowers contributions can be made, to St Matthew's Catholic Church, 607 Stoney Creek Dr, Dauphin, PA 17018, Attn: Knights of Columbus, Matthew Council #15351.
