He is survived by: his wife Doris Jean Fornwalt (Swartz); his children, Debra Zentmeyer and Scott Fornwalt; his grandchildren, Jessica Nelson (Aaron), Kathryn Zentmeyer (Dan), Mollie, Olivia and Nicholas Fornwalt (Hannah); his great grandchildren, Andrew, Logan and Emma Nelson; his great great grandchild Riata Fornwalt; and his great grandchildren, Oscar, Copper and Tillie.
