He is survived by: his girlfriend Mary Gustin; his brother Ronald Matoushek (Connie); his children, Diane Kearns-Puckhaber (Gary) and Mark Matoushek (Renee); his grandchildren, Dezerae (Anthony), Cameron (Kaylee), Charlene (Andrea), Millie, Cory and Tori (Diamante); and his great grandchildren, Travis, Maci, Carson, Elijah, Alanah and Noah.
