He was predeceased by : his parents, William A. Hayward and Elsie Hayward (Kizer); his wife Helen M. Carns; and his siblings, Kay Chieppor and Thomas Hayward.

He is survived by : his son William J. Hayward (Lynn Ann) of Osceola Mills, PA; and his grandchildren, Breana, Hannah, Olivia and Jason. He is also survived by numerous nieces and nephews.
