He was predeceased by: his father Sheldon D. Wormuth II. He is survived by: his mother Frances M. Dell (Baldwin) of Brookville, PA; his daughters, Shasta Wormuth (Keith) of Grampian, PA, Shannon Shape (Branden) of Curwensville, PA, Miranda Wormuth (Anthony Sones) of Hyde, PA, Shelby Clouser (Kyle) of Hyde, PA and Nichole Wormuth (Kenneth Billotte) of Grampian, PA; his sister Branda Jedrzejek (Michael) of Johnstown, PA; and his nephew Craig Lines.
Visitation will be held on Saturday, June 7th 2025 at 10:00 AM at the Heath Funeral Home & Cremation Services (701 Lingle St, Osceola Mills, PA 16666). A funeral service will be held on Saturday, June 7th 2025 at 12:00 PM at the same location.
