He was predeceased by: his parents, John Lewis and Annie Lewis (White); and his siblings, Joseph, Jack, Tom, Jerry, Henry, Allen, Bob, Helen, Mary Lou and Mae.
He is survived by: his wife Carolyn Lewis (Emerick); his children, John Lewis, Raymond A. Lewis (Karen) and Julie Chappell (Kevin); his granddaughters, Chloe and Abigail Lewis; and his sister B. Geraldine Felker. He is also survived by numerous nieces and nephews.
