It is with great sadness that we announce the death of Thomas Andrew Hazenstab of Jackson, Pennsylvania, born in Beaver, Pennsylvania, who passed away on June 9, 2023, at the age of 55, leaving to mourn family and friends. He was predeceased by: his parents, Jerome Hazenstab and Helen Hazenstab; and his brother Jerry Hazenstab. He is survived by: his wife Susan Hazenstab; his sons, Raymond Hazenstab and Tyler Hazenstab; his siblings, Kathy Hazenstab (Nancy Griffin), Bill Hazenstab (Debbie) and Jim Hazenstab (Rhonda); his nephew Brett Hazenstab (Pat); and his niece Stacy Conlin (Rick). He is also survived by numerous aunts, uncles, nieces, nephews and cousins.
