He is survived by: his daughter Jessica Klink of Johnsonburg; his grandchildren, Lillian Rose and Roman Klink; and his brothers, Charles H. Polaski of Johnsonburg, PA, William Polaski of Johnsonburg, PA, Philip Polaski of Johnsonburg, PA and Timothy Polaski of Johnsonburg, PA. He is also survived by several nieces, nephews, great-nieces and great-nephews.
