He is survived by: his brother Robert Slezak (Sharon); his nieces, Jennifer Slezak and Amy Egers (John); his nephew Matthew Slezak (Jenna); and his great-nieces and great-nephews, Blake Cassick (Callie), Coby Cassick, Dominic Egers, Ava Egers, Sophia Egers, Camden Slezak and Kyla Slezak.
