He was predeceased by: his parents, Michael Kozak and Elizabeth Kozak (Kasic); his brother Kenny; his sister-in-law Mary Jane; his nephew Matt; and his brother-in-law Richard Dunn.
He is survived by: his wife Patricia Dunn; his children, Debbie O'Gorman (Gary), Michael, Susan Maxey (Mike), Lober (John), Chris (Lisa) and Rick (Kimmie); and his brothers, Danny, Dave (Alexis) and Marty.
In lieu of flowers, donations can be made to St. Jude Children's Hospital or the Janet Weiss Hospital in Danville in honor of his granddaughter Anna.
