He was predeceased by: his son Philip Cook. He is survived by: his wife Louella (O'Kane); his children, Pam Bervinchak (Kevin) and Todd Cook; and his grandchildren, Jocelyn, Ryan, Jordan, Krista, Karly, Paige, Cyrus, Claudia, Sean and Gabby. He is also survived by 10 great grandchildren.
