He was predeceased by: his parents, John Forlino (Jake) and Mildred Forlino; and his brother John Forlino. He is survived by: his wife Jackie; his children, Jay II, Kevin Forlino (Corrine) and Abby Forlino; his grandchildren, Luna and Jay III; his sister-in-law Nancy; his niece Erika; and his pet Rusty. He is also survived by cousins, nieces, nephews, and friends.
