He was predeceased by: his parents, John Burkhart and Bernadine Burkhart (Crum); and his sister Carol Burkhart Bozsik. He is survived by: his girlfriend Susan Brown; his children, Kelly Ann Burkhart, John Burkhart (Kelly), Ryan Maslak and Shane Breuers (Tanya); his siblings, David Burkhart (Lori) and Karen Burkhart Jordan; his grandchildren, Jonathan, Jaden Burkhart, Cale, Stella Breuers, Dawson, Destiney and Braxton Maslak; and his nieces and nephews, Brandin Bozsik (Sarah), Emily Holbert (Derek), Steve Cotton, Tracy Goebel (Bill), Katelyn Burkhart Baker (Tim) and Meagan Bozsik (Andrew Whims). He is also survived by his children, grandchildren, and dogs.
