He is survived by: his girlfriend Sherry Harrison; his daughters, Brandi Potoka (Kenny) and Brittany Cook; his grandchildren, Megan Potoka, Kennedy Stevenson, Mason Potoka and Jordyn Stevenson; his brothers, Douglas Cook (Debra), Donald Cook (Misty) and Dennis Cook (Darlene); his sister-in-law Joanne Cook; and the mother of his children Debora Cook. He is also survived by numerous nieces and nephews.
