He is survived by : his wife Cathy Kozlowski (Cole); his sons, Zachary Kozlowski of North Versailles and Leo Kozlowski (Lindsey) of Pittsburgh; and his siblings, Barbara Benson (Bernard), Henry and Timothy Kozlowski. He is also survived by nieces and nephews.
