Hetrick Jr. and David Derr of Niles, OH; his grandchildren, Justin Middleton (Kaci), Jonathan Middleton, Kayla Eisler (Jared), Kenneth Jones and Sam Katzman; his brother Charles Hetrick of Conneaut Lake, PA; and his great grandchildren, Jade Middleton, Maxamus Middleton, Lainey Middleton and Emmett Eisler.
