He was predeceased by: his mother June Hoffman; and his sister Donna L. Ash. He is survived by: his children, Douglas J. Hoffman, Amy J. Hoffman and Erin M. Houser; and his siblings, Darrell J. Hoffman (Melissa A.) and Roxann R. Wright. He is also survived by several nieces, nephews, cousins, and other extended family.
