He was predeceased by : his parents, Carmine "Mickey" Forte and Sandra Dudgeon Forte of Centerville. He is survived by : his daughter Alexis Forte-Wadsworth; his son-in-law Danny Wadsworth of California, PA; his daughters, Brooke Forte and Brandee Forte of Bentleyville; his grandchildren, Ava, Jayden and Carmella Wadsworth; his brother Jeffrey Forte (Kathleen) of Washington Township; and his nieces and nephews, Brandon, Caitlyn and Autumn Forte.
