He was predeceased by: his parents, Matthew Kozak and Gladys Wiltrout Kozak; and his siblings, Wilma Kasiewicz, Wilbert Kozak, Wilda Kozak and Marshall "Wayne" Kozak.
He is survived by: his wife Alberta Wojciak Kozak; his son Donovan W. Kozak (Staci); his grandchildren, Hannah and Landon Kozak; and his sisters-in-law, Carolyn Yanuck, Margie Wojciak and Bonnie Kozak. He is also survived by numerous nieces and nephews.
Visitation will be held on Thursday, October 16th 2025 from 10:00 AM to 12:00 PM at the Galone-Caruso Funeral Home Inc. (204 Eagle St, Mt Pleasant, PA 15666). A funeral service will be held on Thursday, October 16th 2025 at 12:00 PM at the same location. A funeral Mass will be held on Thursday, October 16th 2025 at 12:30 PM at the St. Pius X Roman Catholic Church (224 Summit St, Mt Pleasant, PA 15666). A burial will be held on Thursday, October 16th 2025 at 1:30 PM at the St. Pius X. Parish Cemetery (224 Summit St, Mt Pleasant, PA 15666).
