She was predeceased by: her parents, Michael Szamjak and Bertha Szamjak (Pastor) of Homestead, Pennsylvania.; and her brother Stephan Somiak (Tillie). She is survived by: her siblings, Michael "James" Summers, Andrew Somiak (Nancy) and Anna Nagy (Louis); her husband Raymond Hornak; her sons, Mark Hornak (Beth) and Matthew Hornak (Kay); her grandchildren, Sarah Zuger (Jeremy), Faith Hornak, Andrew Hornak (Anna), Samuel Hornak, Rachel Hornak-Gunn (Neil Gunn), Rebecca Hornak, Mary Hornak and Matthew Hornak; and her great grandchildren, Liam, Elsie Zuger, Adam, Kaila, Mason Keesecker, Oliver and Cooper Gunn.
