He was predeceased by: his parents, Francis Cook and Ethel Cook. He is survived by: his wife Tonie (Motta); his children, Tracie Stiffler (late Bob), Shelly Kloc (Dan) and Jeff Cook (Michelle); his grandchildren, Ashley Brinkley (Brian), Lauren Buckwalter (Clayton), Matthew Stiffler, Jacob Kloc (Hope), Dylan Cook and Savannah Cook; his great grandchildren, Raegan, Roby and Blake; and his sister Carol Daw (late Merle). He is also survived by nieces & nephews.
