He was predeceased by: his parents, John Paul and Clara Slater (Ivory); and his siblings, Patricia Baran, Thomas and Daniel Slater.

He is survived by: his wife Joan Slater (Freeman); his children, John P. Slater (Veronique), Jean Gelhaus and Janice McLain (Ronald); his grandchildren, Wendy Gehaus (Michael Davis), John B. Gelhaus, Anna McLain (Jan Horvat), Cara, Olivia, Patrick, Jacob, Bernadette, Gabriel and Genevieve McLain; his great grandchild Braelynn Davis; and his sister Elizabeth Sibol.

Jack's family has requested those wishing to attend please wear a mask and social distance as appropriate.

In lieu of flowers, memorial contributions may be made in memory to: St. John the Baptist Catholic Church at the above address.
