He was predeceased by: his parents, Charles Kusiolek and Anthonette Kusiolek (Kuniec). He is survived by: his brothers, Chuck (Jenny), Kevin (Judie) and Ken Kusiolek. He is also survived by many nieces, nephews and great nieces and nephews.
