She was predeceased by: her parents, Robert Cardot and Gertrude Woody Cardot. She is survived by: her husband Dale J. Formanski; her son Nicholas Formanski (Jamie) of North East; her grandchildren, Jaycee, Brantley and Brooklyn; and her sister Martha Greiner (Ken) of Florida. She is also survived by several nieces and nephews, and great-nieces and great-nephews.
