He was predeceased by: his son Andrew Jr.; and his mother Donna Milchalk (Urban). He is survived by: his wife Lori Milchalk (Fawcett); his daughter Angela Pouch (Michael); his grandchildren, Michael and Charlotte; his siblings, DeAnna Moore (late Willi J.), Danelle White (Anthony) and Denise McCullough (Scott); and his father Norman Milchalk. He is also survived by many loving nieces and nephews.
