He was predeceased by: his parents, John L. DeFazio and Marian C. DeFazio. He is survived by: his grandson Amedeo J. DeFazio; and his former wife Charlene DeFazio. He is also survived by six siblings, an uncle, a cousin and friend.

In lieu of flowers, the family will be accepting donations benefiting the youth hockey generations of the future.
